Providing us with a comprehensive list of the signs and symptoms the patient is experiencing is crucial in helping our interpreting radiologist provide an accurate diagnosis. It also allows us to demonstrate medical necessity. Please avoid using “R/O”, “Possible”, or “Probable” for reasons for the exam, and avoid blanket codes.
For patients with complex medical/surgical problems, please fax copies of recent office notes, operative reports, and/or hospital admission/discharge summaries.
